Regardless of the method, a hair transplant involves two key steps. Extraction and implantation, let’s break them down.

Extraction – Removing hair follicles from the donor area.
Implantation – Placing those follicles into thinning or balding areas.

At Hair & Skin Science, we use FUE extraction combined with Direct Implantation, with follicles stored in HypoThermosol® between steps to improve survival rates.

Extraction

Follicular Unit Transplantation
FUT - The Strip Method

A strip of scalp is surgically removed from the back of the head, and individual follicles are then dissected under a microscope before being implanted.

Pros: Can yield a high number of grafts in one session, typically used for larger procedures.

Cons: Leaves a linear scar, requires stitches, and has a longer recovery time. This method is seen as outdated in the hair transplant industry.

Extraction

Follicular Unit Extraction
FUE - The Modern Method

Individual hair follicles are extracted directly from the donor area using a small punch tool, leaving tiny, dot-like scars instead of a single linear scar. These follicles are then implanted one by one into the recipient area

Pros: FUE is the most advanced and widely used extraction method, offering natural results with minimal invasiveness, no stitches, no linear scarring, and a faster healing process.

Cons: More time-intensive than FUT and requires a skilled team to avoid over-harvesting and preserve follicles.

What About ‘Micro-FUE’?

Micro-FUE is just a marketing term, it’s still FUE. Some clinics use ‘Micro-FUE’ to describe using smaller punch tools, but the process remains the same.

Implantation

Slits Implantation Method
Less Density, More Trauma

Tiny slits (small incisions) are first created in the recipient area using a specialised scalpel, then the extracted follicles are manually placed into these slits using specialised tweezers.

Pros: The doctor has full control over the angle, depth, and direction of the implanted hair.

Cons: If too many slits are made too close together or with too much force, it may impair blood flow and potentially lead to necrosis in the surrounding tissue. The slits need to be placed further apart than direct methods, reducing density and causing more trauma to the scalp, which may reduce follicle survival rates.

Implantation

Direct Implantation Method
The Modern Standard

Instead of creating slits first, a specialised tool (such as a Choi-Pen or Multi-Device) is used to simultaneously create the incision and insert the follicle in one motion.

Pros: This method allows for higher-density implantation, as the follicles can be placed closer together. Additionally, less trauma to the scalp can result in higher rates of follicle survival.

Cons: While direct implantation is the gold standard for implantation methods, like all methods, the results depend more on the doctor’s skill than on the tool itself.

What About ‘DHI’ or Other Terms?

Some clinics use branded terms like ‘DHI’ to market their method as unique, but these are standard techniques, rather than new procedures. ‘DHI’ is a technique where a Choi-Pen or similar device is used for implantation, which falls under direct implantation.

Any other acronym being marketed to you is just that, marketing. The most critical factor in achieving great results isn’t a different name, it’s the expertise and precision of the medical team performing your procedure.

What to Expect During Recovery

An FUE hair transplant is a minimally invasive procedure, but like any medical treatment, it comes with a healing process. While there is little to no downtime, the journey from transplant to full results happens gradually over the course of a year as hair grows in cycles.
